• Assistance Animal Registration Form (document to be completed before the start of fall semester by students with assistance animal accommodations)
• Residential Accommodation Documentation Request Form (document to be completed by the health care provider. Covers housing, dietary, assistance animal, and parking exemption accommodations)

Student Academic Forms: (You have already met with our office and set up academic accommodations)

• Schedule an Exam (UNDERGRAD STUDENT)
• Request Accessible Ebook (UNDERGRAD, GRAD, and LAW STUDENT)
• Request for Accessible Classroom Furniture (UNDERGRAD, GRAD, and GRAD STUDENT)
• Notetaking-Audio Recording of Lectures (UNDERGRAD, GRAD, and LAW STUDENT) (document should be uploaded to student file through Intake Form once approved by professor that this accommodation is approved to use)
