Welcome Letter - Graduate School
On behalf of the Student Health Center, welcome to Lewis & Clark College!
We want to promote your health as a student, along with the health of our community. In accord with this, we have four health considerations that must be addressed soon. Please complete the following:
Please note that you must set up your Lewis & Clark email account and then wait at least 24 hours before you can set up your portal account. To set up your account, follow this link http://lclark.medicatconnect.com and click “Register” in the upper right hand corner. The portal is a secure platform that will allow you to enter immunization and other health information described below. If you have any difficulties setting up your portal account, please check out this help sheet.
Required of each student:
- M.M.R. (Measles, Mumps, Rubella) immunization. Two doses are required at least 28 days apart and after 12 months of age. Alternatively, an MMR lab (Titer) report indicating immunity satisfies the measles requirement. Please enter your two MMR immunization dates in the Health Portal. Click on Immunizations in the left column to enter your dates (you may also enter your other immunization dates, but they are not required). For additional information, see Oregon law 333-050-0130.
- Tuberculosis testing is required if you meet either of the following criteria: you were born in a country with a high incidence of tuberculosis disease (“TB”), or in the last five years, you lived in, traveled to, or emigrated from a country with a high incidence of tuberculosis disease. For a list of countries with high incidences of tuberculosis disease, click here. No matter your country of birth or recent travel history, complete the TB Screening Questionnaire on Medicat by clicking on the Forms button in the left column and selecting TB Screening Questionnaire under Required Health Forms for New LC Students. If your answers indicate you were born in a country or recently visited a country with a high incidence of tuberculosis, you will need to receive a TB test (e.g. Mantoux tuberculin skin test, Interferon Gamma Release Assay, or chest x-ray) and submit the results under Immunizations.
Note: In accordance with Oregon law, Lewis & Clark College requires all full-time students to have two doses of MMR (measles, mumps, and Rubella) vaccine administered at least 28 days apart and after the student is 12 months of age. Students who do not meet the Oregon law requirements may experience registration holds or cancellations.
You may need to collect medical information from your family or healthcare provider in order to complete these forms.
Students in the Graduate School of Education and Counseling (GSEC) are not required to have health insurance, but it is strongly encouraged.
Lewis & Clark has partnered with PacificSource to offer a health insurance plan for students that contains robust benefits that support the health of college students. The plan provides excellent value as well as access to a wide network of providers both in Portland and across the nation. The plan may provide a better value for you than health insurance offered through a parent’s or partner’s employer. We encourage you to review our school-sponsored insurance plan and see if it meets your needs.
GSEC students who wish to enroll in coverage must do so at the start of each and every semester via the PacificSource Online Enrollment System. The enrollment window opens each semester about one month prior to the policy period. GSEC students must enroll within 31 days from the effective date of coverage each semester. It is the student’s responsibility to purchase coverage each semester by the enrollment deadline.
Oregon state law requires us to provide ALL students with information on vaccine-preventable diseases known to occur in individuals between 16 and 21 years of age. We encourage you to read that information at this webpage, and then talk with your health care provider about what additional vaccines might be appropriate.
